Medical: ‘Experts’…. Look At Civilians Hurt Supporting War

     The most prevalent diagnoses for civilians were musculoskeletal/spine injuries (19 percent), combat-related injuries (14 percent) and circulatory disorders (13 percent). Among members of the military, the most common diagnoses were musculoskeletal (31 percent, 6.4 percent of them considered war-related), combat (14 percent) and psychiatric (9 percent).

     Cohen noted that civilians with psychiatric diagnoses were significantly more likely to return to duty (16 percent, versus 9 percent for soldiers). “Despite the military’s emphasis on screening and early treatment for psychiatric disorders, they still take a much greater toll on military personnel than nonmilitary personnel,” said Cohen, who is also director of Chronic Pain Research at Walter Reed Army Medical Center.

 

    After looking through this, I had some objections with the study. It is lumping in civil servants or federal employees with non-federal employees or civilian contractors.  The reason why I disagree with this combination is that federal employees would have different motivations and different compensations than civilian contractors. It would have been better to completely separate the two.

    Dr. Cohen said this as well–‘Civilians, who often work in security and transportation jobs, are less likely to be in the line of fire and don’t expect to be injured in combat‘. I absolutely disagree with this statement and I am not sure how he came to this conclusion?  Even the KBR truck drivers that were hired to work in Iraq at this specific time line of the study (2004 to 2007) would have had to have known through the news and through word of mouth, that they were signing up for a very dangerous contract in an active war zone.

    The security contractors that worked at that time, and especially in Iraq, all considered the idea of being ‘injured’ or killed in combat every day they worked there.  How could a person in this position not consider this?

    This study also highlights some strengths and weaknesses of the contracting model in war zones, as it pertains to the medical side of things.  It shows how contractors view their job as a profession that will help to feed their family and pay the bills. The study makes no mention of that contractor’s patriotism or their desire to be with the team. The various motivations for them to continue going back to that war zone is varied, and this study does not differentiate. This study also represents a very dangerous time period to be a contractor, and a key time period of the development of the industry.

    On the other hand, the study pointed out that after civilians were wounded from ‘combat related’ injuries, they were more likely not to return.  Is that because they lost heart for the work or is that because the injury was debilitating enough to where they could not go back? Does the study make any mention of how many incidents that an individual had been through, both in their military history and contractor history?  Or how many of these folks have returned back to work after such incidents, but years later. This is happening in this industry, and the contracting model allows individuals to do this, dependent upon their resume and vetting.

    The other interesting statistic was this one. ‘Cohen noted that civilians with psychiatric diagnoses were significantly more likely to return to duty (16 percent, versus 9 percent for soldiers).’  This statistic needs to be clarified. How many of these folks that were questioned, were military veterans or police veterans that had carried their mental issues with them into their contract? Or what kind of diagnosis is given for each individual questioned, and was it related to combat and the war, or were these psychiatric issues a carry over from something else going on in their lives?

    With that said, the drive for a contractor with mental problems to continue working to feed their family and pay the bills, might be stronger than seeking help and not working.  At this time period of the study, a secret clearance was not mandatory. But there was the infamous CRC that many contractors had to cycle through at that time, and the military was tasked with medically screening folks.  At the bases that conducted this screening, contractor’s medical backgrounds were reviewed.

    Even with these screening processes, contractors still slip through.  Danny Fitzsimons is just one case of a contractor with mental issues making it through the system. There is also the peer review or on the ground management of teams that helps to screen folks as well. If there is an individual that is mentally unstable, they will be identified and removed from contract due to their liability. Everyone has to be a little crazy to do this work, but no one wants to depend their survival on some dude that is mentally gone.

     The high musculoskeletal/spine injuries statistic is the one statistic that was intriguing to me.  With the use of body armor and how heavy it is, as well as the hours of standing around or driving around wearing it, this can have adverse effects on the body.  Even though the armor can save a life, it still can injury a person just because of it’s weight. This is a problem for the military, and for contractors, and back injuries and the pain medications required to deal with those injuries will become very common place as contractors and military continuously work in war zones and wear this stuff. Armor is a paradox of sorts, and not to mention it’s limitations on the mobility of a war fighter. It may save your life, but it will also allow enemy combatants to out run you and give them a chance to fight again.

     Now what would be an interesting study is to pick up where they left off and see how things look now(2007 to 2011)?  A lot has happened since then.  If the study was better targeted and consultants outside of the medical group were asked to help guide the process, then that would be a more thorough and respected study. People will support what they help to create…. –Matt

February 21, 2011By Stephanie DesmonAfter analyzing data on 2,155 private contractors, diplomats and other civilians supporting war efforts in Iraq and Afghanistan who were medically evacuated from combat zones, researchers have found that such civilians are more likely to be evacuated for noncombat-related injuries but more likely to return to work in-country after treatment for these conditions.

Still, the findings of the Johns Hopkins–led research team, published online in CMAJ, the journal of the Canadian Medical Association, note that 75 percent of the nonmilitary group medically evacuated from the war zones to Landstuhl Regional Medical Center in Germany between 2004 and 2007 did not return to the field.

SUBJECT: COMISAF’s Counterinsurgency (COIN) Contracting GuidanceDownload Official Release

The scale of our contracting efforts in Afghanistan represents both an opportunity and a danger. With proper oversight, contracting can spur economic development and support the Afghan government’s and ISAF’s campaign objectives. If, however, we spend large quantities of international contracting funds quickly and with insufficient oversight, it is likely that some of those funds will unintentionally fuel corruption, finance insurgent organizations, strengthen criminal patronage networks, and undermine our efforts in Afghanistan.

In view of these points, contracting has to be “Commander’s business.” Indeed, I expect Commanders to consider the effects of our contract spending and understand who benefits from it. We must use intelligence to inform our contracting and ensure those with whom we contract work for the best interests of the Afghan people. We must be better buyers and buy from better people. Consistent with NATO and national contracting laws and regulations, we must:

Understand the role of contracting in COIN. Purchases we make for construction, goods, and services can bolster economic growth, stability, and Afghan goodwill toward their government and ISAF. Contracts with Afghan firms that procure Afghan goods and services generate employment and assist in the development of a sustainable economy. However, if we contract with powerbrokers who exclude those outside their narrow patronage networks or are perceived as funneling resources to one community at the expense of another, the effect on Afghan perceptions and our mission will be negative. Thus, we must incorporate COIN Contracting topics into training for Commanders.

I have just received a confirmed report there was a a September 6, 2010  attack on a forward operating base Howz-e-Madad in Kandahar province Afghanistan. This attack took the lives of three DynCorp LOGCAP IV employees and wounded several others.

Dyncorp just released this:

September 7, 2010 – We are deeply saddened to report that a September 6 attack on a forward operating base in Howz-e-Madad, in Kandahar province, Afghanistan, took the lives of three LOGCAP IV personnel and wounded several others.

William Allen, 51, of Munhall, Pennsylvania, worked as an electrician on the LOGCAP IV program and was killed at the time of the incident. Alan Herzel, 48, of Milton, Florida, who worked as a plumber foreman, received medical care after the incident but later passed away as a result of his injuries. A third subcontract employee who provided information technology support for the program also lost his life in the attack; DynCorp International will defer to his employer to release any personal details.

A number of individuals who were wounded in the attack were transported by medevac for hospital care.

In a condolence note to LOGCAP IV management, Colonel Cameron G. Holt, Defense Contract Management Agency (DCMA) Commander, Afghanistan, wrote:

“As is too often the case in war, the cost of freedom extends not just to the Soldiers, Sailors, Airman, and Marines who have pledged their lives to defend their country; it is also borne by our civilians at home and among the ranks of American defense contractors like you who stand shoulder to shoulder with us in this struggle.”

Our thoughts and prayers are with those who were injured or killed, with their families, and with all of our colleagues at FOB Howz-e-Madad.

Former Valley Soldier Killed in Afghanistan

HAVANA – The family of a former valley soldier is mourning tonight.

Lots of questions surround the death of UTPA alumn Javier de la Garza Jr.

His grandparents in Havana tell CHANNEL 5 NEWS he worked as a private contractor overseas.

De la Garza was killed during some kind of attack in Afghanistan earlier this week.

The 27-year-old lived in Austin for some time before joining the Army in 2000. He deployed to Iraq then settled in the Valley after his service. He attended UT-Pan American in Edinburg before deciding to return overseas. This time he went as a civilian and worked in the IT department for a private contractor in Afghanistan.

De la Garza’s family hasn’t been told when his body will return to the states. (click HERE for original article)

Afghanistan: The Future Of Training–More Money, More Demand For Trainers

The United States expects to spend about $6 billion a year training and supporting Afghan troops and police after it begins withdrawing its own combat troops in 2011.

The estimates of U.S. spending through 2015, detailed in a NATO training mission document, are an acknowledgment that Afghanistan will remain largely dependent on the United States for its security.

That reality could become problematic for the Obama administration as it continues to seek money for Afghanistan from Congress in a time of increasingly tight budgets.

In Brussels, a NATO official said Monday that alliance commander Gen. David Petraeus had asked for 2,000 more soldiers, with nearly half to be trainers for the rapidly expanding Afghan security forces. The NATO official requested anonymity because he was not authorized to speak on the subject.

    A couple of things with these stories. Will Petraeus get these extra troops if NATO cannot scrape the bucket and get them on the scene?  Because politically and economically, sending more troops to Afghanistan is a hard sell in all the countries participating, and not just the US. How will this look during the coming elections in the US if more troops are asked for on top of the already thousands of troops requested for the surge?  Hell, some countries are backing out last minute (like the Dutch) and these actions always cause ripples in the planing and operations of the war effort.  So as I have said before, as NATO falters, contractors will be taking up the slack.

   I also continue to see plenty of training job ads, and according to the second article below, the money will be there for training in Afghanistan well into the future. The only thing though is politically, this administration and probably the next will continue to have problems with sending troops or getting congress to sign off on that training money.  As the troops get more limited in presence, and money becomes more scarce, contractors will continue to be an important tool to execute these training duties.

    The other thing I could see happening is more hybrid training programs popping up.  Where military folks mixed with contractors will be used to train the Afghans. We already see a little bit of this, and I think this combination works because the customer (US government) can ensure there is military oversight on these projects. It would also ensure that there is consistency in the training programs, and a metrics can be maintained by the military itself.

    All of this is just speculation, but given what is already on the ground, the military and contractor relationship will become even more important as time goes on. –Matt

NATO eyes 2,000 extra troops for Afghanistan: official

US General David Petraeus, the commander of the war in Afghanistan, has requested 2,000 extra troops to bolster a crucial mission to train Afghan security forces, a NATO official said Monday.

The mission would come on the heels of the deployment of tens of thousands of soldiers who were sent as part of a surge strategy aimed at crushing a resilient Taliban insurgency, the official said.

“There is now a discussion under way for additional resources, principally trainers, that could be sent to Afghanistan to bolster the mission,” said the official, who requested anonymity.

At least 750 of the new soldiers would focus on training Afghan forces, he said, refusing to give more details about the rest of the mission. He said it was premature to say when the 2,000 extra troops would be deployed.

Getting Afghan security forces trained so they can take over security responsibilities is a paramount condition for the withdrawal of foreign troops from the nation, worn down by war.
